Requesting Your Health Care Records

Clients of In Bloom Therapy have the right to request access to their health care records.

To request your records, please:

• Submit a request via email to your clinician or through the secure client portal

• Complete a Records Request form via your Simple Practice Portal

• Complete identity verification, if required

• Be aware that reasonable fees related to cost-based expenses such as copying, supplies, or secure delivery may apply as permitted by law

• Records will be provided within 15 days of written request
