Requesting Your Health Care Records

Clients of In Bloom Therapy have the right to request access to their health care records.

To request your records, please:

• Submit a written request via email to dsalinas@inbloomtx.com or through the secure client portal

• Include your full legal name, date of birth, and a clear description of the records requested

• Complete identity verification, if required

• Be aware that reasonable fees may apply as permitted by law

• Records will be provided within the timeframe required by Texas law.
